Subtitle A of Title V of the Gramm-Leach-Bliley Act ("GLBA") has privacy provisions relating to consumers' financial information. Under these provisions, financial institutions have restrictions on when they may disclose a consumer's personal financial information to nonaffiliated third parties. To that end, this form may contain confidential material for the sole use by the intended recipient(s). Any review, use, distribution or disclosure by the intended recipient(s) is strictly prohibited. The information contained herein is intended to be used solely for the ordering of real estate information. Any other use of the information is also prohibited.
